Quick answer

Choose the transaction in the form finder below, open the current official HSMV PDF and read every instruction page before signing. A form does not prove eligibility by itself; the office may require identity, ownership, tax, lien, VIN or certified supporting documents.

Working official form finder

Find a Florida HSMV Vehicle Form

Select a transaction to see commonly associated official forms and preparation notes. Nothing is submitted or stored.

How to Use the Form Finder

Select the actual transaction, not the result you hope to receive. The tool shows commonly associated official forms and planning notes. It runs entirely in your browser and does not collect names, plate numbers, VINs or document images.

Always Use the Current Revision

Florida revises forms and procedures. Download from an official FLHSMV domain shortly before filing. Do not reuse a saved blank PDF from an old transaction without checking the revision date and current instructions.

Signature and Identity Rules

Owner, co-owner, purchaser, seller, lienholder, dealer, agent and attorney-in-fact signatures are not interchangeable. Some transactions allow a power of attorney; others require the person or a specially worded authority. Do not pre-sign blank forms.

Originals, Certified Copies and Plain Copies

The procedure determines whether an original, certified copy or plain copy is acceptable. A phone image may help an office identify the issue but may not satisfy filing rules. Keep a complete copy of everything submitted.

Notarization and Odometer Cautions

Notarization does not cure false information or missing ownership. Odometer and VIN statements carry legal consequences. Complete them from the vehicle and source documents, not from memory or an online listing.

Before You Submit

  • Match names exactly to identification and ownership records.
  • Use permanent dark ink where required.
  • Complete every applicable field and mark non-applicable fields as instructed.
  • Attach tax, lien, VIN, insurance and authority evidence.
  • Confirm the office handles the transaction and accepted payment.
